Terms & Conditions - the basics

Project 15:11 Ministries is a Christ-centered nonprofit ministry committed to serving individuals and families experiencing genuine need.

To help us serve our community fairly and responsibly, all assistance requests are reviewed using consistent program guidelines.

Eligibility and Approval

Submitting an application does not guarantee assistance. Requests are considered based on:
 

  • The type and circumstances of the need

  • Whether the request falls within the scope of one of our assistance programs

  • Program guidelines and assistance limits

  • Available ministry funds, resources, and volunteers

  • Our ability to reasonably verify the request

  • Safety and other practical considerations
     

Project 15:11 Ministries reserves the right to request additional information or reasonable documentation before approving assistance.

Fair and Consistent Review

Assistance decisions are based on demonstrated need, program eligibility, available resources, and the ministry's ability to meet the request.
 

Project 15:11 Ministries does not make assistance decisions based on race, color, national origin, sex, age, disability, religion, religious belief, or lack of religious belief.
 

Project 15:11 Ministries is a Christian ministry and may offer prayer, encouragement, or faith-based materials. Participation in prayer or other religious activities is voluntary and does not affect eligibility for assistance.

Verification of Information

Applicants must provide accurate and truthful information.
 

Project 15:11 Ministries may reasonably verify information submitted in an application.
Depending on the type of assistance requested, this may include requesting supporting documentation or contacting a vendor, pharmacy, utility provider, service provider, or other organization connected to the request.
 

Whenever practical, financial assistance may be paid directly to a vendor or provider rather than given directly to the applicant.

Misuse or False Information

Knowingly providing false, misleading, altered, or fraudulent information may result in denial of the current request and may affect eligibility for future assistance.
 

Submitting duplicate applications, attempting to avoid program assistance limits, or intentionally misrepresenting household information or circumstances may also affect future eligibility.

Privacy

Information collected through our assistance applications will be used for the purpose of reviewing, verifying, administering, and documenting assistance requests.
 

Applicants should provide only information reasonably necessary for the ministry to evaluate their request.

Applicant Acknowledgement

By submitting an application, I acknowledge that:
 

  • I have read and understand the Project 15:11 Ministries Assistance Program Policy.

  • The information I provide is true and accurate to the best of my knowledge.

  • I understand that submitting an application does not guarantee assistance.

  • I understand that Project 15:11 Ministries may request additional information or documentation and may reasonably verify information related to my request.

  • I understand that false or misleading information may result in denial of assistance or affect my eligibility for future assistance.
